Services can be accessed through self-referral or third-party referral utilising the following approaches:

  • Contact Daley Connections via:

  • Discuss with your Local Area Coordinator (NDIS Participants)

  • Engage with your Support Coordinator (NDIS Participants)

  • Reach out on our Social Media platforms​

The NDIS self-directed funding model allows Participants control over major decisions including which service provider or services you require. Private clients have access to the same services as NDIS Participants.

 All referral enquiries will be responded to within 48 hours of initial contact. This includes private clients.

 

Our director at Daley Connections will arrange a time and place to meet with you and discuss your needs, your goals and how our services could support you/your child to achieve their goals.
